In this pivotal role, you will be responsible for coordinating patient care, ensuring that individuals receive the appropriate medical services, and supporting them throughout their hospice journey. Responsibilities Assess and evaluate patients’ needs to develop comprehensive care plans. Coordinate with multidisciplinary teams to ensure effective patient management. Monitor vital signs and provide direct patient care as needed. Advocate for patients by communicating their needs and preferences to healthcare providers. Conduct regular follow-ups to assess the effectiveness of care plans and make necessary adjustments. Utilize knowledge of physiology to educate patients about their conditions and treatment options. Implement strategies for effective utilization management to optimize patient outcomes. Experience Current Registered Nurse (RN) license required. Familiarity with vital signs monitoring and patient assessment techniques. Knowledge of utilization management principles is advantageous. Strong communication skills and the ability to work collaboratively within a healthcare team. Experience in case management or related fields is a plus.
