M-F 8am-4pm

Pay Range:

$60-65 hourly, plus mileage reimbursement Registered Nurse•

Hospice Responsibilities:

Perform patient assessments to determine hospice eligibility and care needs. Develop and revise individualized patient care plans. Ensure efficient use of hospice services and maintain visit productivity standards. Accurately document admissions, discharges, and re-visits using EMR within required timeframes. Provide age-appropriate education to patients and families, updating plans as needed. Manage assigned caseloads effectively with clear prioritization and interdisciplinary coordination. Supervise home health aides per standards and ensure safe medication practices, including reconciliation and order verification. Participate in IDT meetings bi-weekly, and contribute to performance improvement initiatives. Attend monthly staff meetings, assist with new employee mentorship, and support agency growth through cross-training and additional assignments. Registered Nurse•

Hospice Qualifications:
