The ideal candidate will possess a strong background in wound care management, demonstrating expertise in assessing, planning, and implementing effective treatment strategies for patients with complex wounds. This role requires excellent clinical skills, attention to detail, and the ability to work collaboratively within a multidisciplinary team to ensure optimal patient outcomes. Responsibilities Conduct thorough assessments of patients’ wounds and overall health status. Develop individualized care plans based on assessment findings and best practices in wound management. Administer appropriate wound care treatments and interventions, including dressing changes and infection control measures. Educate patients and their families on wound care techniques, prevention strategies, and self-management practices. Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate comprehensive patient care. Maintain accurate documentation of patient assessments, interventions, and outcomes using electronic health records (Epic). Stay current with advancements in wound care practices and participate in ongoing professional development. Requirements Valid nursing license (RN or LPN) in the state of practice. Experience in long-term care or hospice palliative medicine preferred. Knowledge of anatomy and physiology as it relates to wound healing. Familiarity with tube feeding procedures is a plus. Previous experience in emergency room (ER), pediatrics, geriatrics, or hospital medicine is advantageous. Proficiency in coding for wound care documentation is desirable. Strong communication skills and the ability to educate patients effectively. Commitment to providing compassionate care to diverse patient populations.
