The Dialysis HomeTherapies Nurse coordinates the administrative and clinical aspectsof the home dialysis program, including home hemodialysis andperitoneal dialysis treatment options, and communicates withpatients and support staff to ensure the highest standards of care.

Schedule:

Fulltime, Monday through Friday, shifts start at 8am; oncall as needed

ResponsibilitiesWhat You Can Expect:

Collaborate with medical team on patient careconcerns, hospitalizations, discharges and/or modality changes,etc. Coordinate home training sessions, clinic visits, patient careconferences, home visits and all clinical and educationalactivities to assure efficient and productive use of nursing hoursand program facilities Monitor patient supply ordering and usagewhile maintaining quality care Communicate regularly regarding homeprogram activities with interdisciplinary care team Monitor ongoingpatient care parameters Perform all registered nurse functions andduties Create and maintain appropriate record keeping, patienttraining and documentation in accordance with ESRD Network,regulatory agencies, DCI CQI program and the dialysis facility’spolicies and procedures
