Salary Range:
$60,028.00 – $99,023.00 The RegisteredProfessional Nurse in the ambulatory setting provides care acrossthe life span to individuals, families, caregivers, groups,populations, and communities. This is accomplished duringface-to-face encounters or through a variety of telecommunicationstrategies, often resulting in the establishment of long-termpatient care relationships. During each encounter, the registerednurse focuses on patient safety and the quality of nursing care byapplying appropriate nursing interventions, such as identifying andclarifying patient needs, performing procedures, conducting healtheducation, promoting patient advocacy, coordinating nursing andother health services, assisting the patient to navigate the healthcare system, and evaluating patient outcomes. Ambulatory careregistered nurses, acting as partners and advisers within theinterdisciplinary team, assist and support patients and families tooptimally manage their health care, respecting their culture andvalues, individual needs, health goals and treatment preferences.
Minimum Qualifications Education:
Graduate of a professionalacademic nursing program in which a Diploma, Associate Degree orBachelor’s Degree is conferred. Licensure, Certification ℜgistration: Must hold current NYS Registered Nurse license.
Obtains and maintains certification in Basic Life Support (BLS).Some departments require additional certifications.
Experience:
Previous nursing experience in an ambulatory care setting or 1 yearnursing experience obtained outside an ambulatory care setting ispreferred.
Skills, Knowledge & Abilities:
Ability to improve jobperformance through continuing education. Possesses knowledge ofquality concepts, principles, and problem-solving tools andtechniques and demonstrates ability to apply in support ofdepartmental and patient care quality improvement.

Assessment:
Collects pertinent data andinformation relative to the patient’s health or situation.
Diagnosis:
Analyzes assessment data to identify patient needs basedon disease process.
Outcomes Identification:
Identifies expectedoutcomes for a plan individualized to the patient.
Planning:
Develops a plan that contains nursing interventions to achieveexpected, measurable outcomes.
Implementation:
Provides therapiesand instruction to address health promotion and prevention ofillness or injury.
Coordination of Care:
Coordinates care with thepatient, family, interprofessional team, and community basedresources to affect safe transitions in accordance with federal andstate regulations, utilizing the chain of command as necessary.
Patient Education:
Delivers patient/family education with respectto culture, values, developmental level, and readiness forlearning.
Evaluation:
Evaluates progress toward the achievement ofgoals and outcomes. Standards of Professional Performance Practicesethically. Practices in a manner that values cultural diversity andthe principles of inclusion. Communicates effectively in all areasof practice. Patient Safety:
Supports a system thatencourages the identification and analysis of errors and nearmisses in the context of a safety culture. As assigned, conductsdepartmental safety inspections to assure compliance with hospitalsafety standards as evidenced by documented inspection reports. Isaware of and compliant with all hospital and regulatory agencysafety goals and objectives
