RN Case Manager – No Weekends! 4.1 4.1 out of 5 stars 385 Watervliet Shaker Road, Latham, NY 12110 Eddy Senior Care is now hiring an RN Case Manager, Full-Time Day shift – No weekends and no on-call hours! Don’t miss out on an exciting opportunity to join our dynamic team! This position is a nice mix of home visits within a 1- 20 mile radius of our location and office time to support care management. As a member of the Eddy SeniorCare team, the RN Case Manager will:
FOCUS ON MANAGING PATIENT CARE
, not on the # of visits per day Work closely with an interdisciplinary team, including MDs and NPs, focused on prevention and achieving the best outcome Build long term relationships with patients/families focused on prevention vs reacting to episodes/crises Eddy SeniorCare is an exciting, innovative model of care for seniors, located at 385 Watervliet-Shaker Road, Latham. Our Program of All-Inclusive Care for the Elderly is located within Provincial House. We provide a team-based model of care including a physician, RNs, LPNs, Home Health Aides, physical/occupation/speech therapists, social workers, transportation, a day center team, and more!
Primary responsibilities include:
Case managing an assigned caseload of participants Completing assessments, providing treatments Orienting LPNs and HHAs to the participant’s plan of care and overseeing LPN and Aide care. Collaborating closely with the interdisciplinary care team.
Work schedule:
Monday to Friday, 8am to 4:30pm
Requirements:
NYS RN Licensure. Experience in home health or a long term care facility preferred but not required. Must possess a passion for caring for seniors and demonstrate a respectful, caring, professional demeanor. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.”
