Position Summary:

We are seekingexperienced Registered Nurses to provide in-home care to adultpatients across the Bronx borough. The RN will manage a caseload of5-8 patients daily, delivering skilled nursing care directly inpatient homes. This position requires clinical flexibility,self-direction, and a passion for community-based care.

KeyResponsibilities:

• Provide direct skilled nursing care inpatients’ homes across the Bronx
• Assess, monitor, and evaluatepatient health status and care needs
• Administer medications andtreatments in accordance with physician orders
• Coordinate withinterdisciplinary care teams to implement and adjust care plans
• Educate patients and families on health maintenance and diseaseprevention
• Accurately document all patient interactions,observations, and interventions
• Ensure compliance with infectioncontrol and safety protocols
• Travel daily within the borough tocomplete scheduled patient visits

Required Qualifications:

• ActiveNew York State RN License
• Current AHA BLS certification
• Minimum2 years of recent acute care experience (Med-Surg, Critical Care,Maternal)
• At least 1 year of current adult home health experience
• Strong clinical assessment and time management skills
• Willingness to travel throughout the Bronx and work a consistentschedule (no overnight shifts)
• Comfortable managing 5-8 cases perday independently
