Renaissance Home Health Care is committed to delivering compassionate, high-quality home health services. We are seeking a dedicated Registered Field Nurse to join our nursing team, providing expert patient assessments and care management in the home setting.
Job Description:
As a Registered Field Nurse, you will assess patient health problems and needs, develop and implement individualized nursing care plans, and maintain detailed medical records. You will also oversee and supervise Home Health Aides and coordinate case management to ensure the delivery of excellent patient care.
Primary Responsibilities:
Conduct comprehensive patient assessments upon admission, including physical, emotional, and environmental factors. Develop and implement personalized nursing care plans aligned with physician orders and patient needs. Maintain accurate clinical documentation, progress notes, and medical records. Monitor patient conditions closely, reporting any changes to physicians or supervisors promptly. Educate patients and their families about diagnoses, treatments, and care responsibilities. Supervise and support Home Health Aides, ensuring quality care delivery. Assist in staff orientation, training, and ongoing education. Manage and update the Plan of Care as needed, including certification and recertification. Perform other duties as assigned to support the nursing department’s mission.
Specialized Skills & Technical Competencies:
Expertise in patient assessment and care planning Knowledge of pathophysiology and medication management (oral, IM, SQ) Familiarity with infection control, aseptic techniques, and catheter care Ability to effectively communicate with patients of all ages and backgrounds Strong understanding of therapeutic diets and medication interactions
Qualifications:
NYS Licensed and currently registered as a Registered Professional Nurse (RN) Baccalaureate Degree in Nursing or related field (desired) Homecare experience (preferred) Why Join Us?
