POSITION SUMMARY•RN Case Manager
POSITION DUTIES
•Utilization Management / Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS
•NY RN license; AHA BLS; 2 years Hospital Inpatient Care Management experience and utilization management; Local Candidates only
PREFERRED QUALIFICATIONS
•3-5 years Care Management experience; EPIC experience; PRI (Patient Review Instrument); SCREEN experience/certification
LENGTH OF ASSIGNMENT
•9 weeks
SHIFT / HOURS PER WEEK
•9:00 AM-5:00 PM M-F (35hrs/wk)
