Week Contract Position Summary:

We are seeking a skilled and compassionate Registered Nurse

• Case Manager role in Bronx, NY .

This 13-week assignment offers an opportunity to contribute to comprehensive, patient-focused care coordination. The ideal candidate will have a strong background in case/resource management, utilization review, and interdisciplinary care planning.

Key Responsibilities:

Conduct comprehensive case management and utilization review assessments Facilitate discharge planning and coordinate post-acute care services Collaborate with interdisciplinary teams to ensure continuity of care Maintain accurate, timely documentation in Electronic Medical Records (EMR) systems (EPIC preferred) Apply PRI and SCREEN tools for patient assessments (preferred) Serve as a liaison between healthcare providers, patients, and community resources

Requirements:

Active RN license in New York State (Primary Source Verified) Current AHA BLS Certification Minimum of 2 years of experience in case/resource management and utilization review Excellent communication, organizational, and critical thinking skills Ability to work independently and collaboratively in a fast-paced care setting

Preferred Qualifications:

3-5 years of experience in care/case management Proficiency with

EPIC EMR

systems PRI and SCREEN certification highly preferred
