You will work closely with clients to assess their needs, develop care plans, and connect them with appropriate resources to improve their well-being. Duties Conduct assessments to identify clients’ needs and develop individualized care plans Provide crisis intervention and support during challenging situations Coordinate services such as housing assistance, healthcare, and counseling Monitor clients’ progress and adjust care plans as needed Collaborate with other professionals, such as social workers and healthcare providers Advocate for clients’ rights and ensure they receive quality care Skills Knowledge of programs like Section 8 and Medicare Experience in crisis management and intervention Familiarity with addiction counseling and motivational interviewing techniques Understanding of behavioral health issues Background in child welfare or childcare services Strong communication and interpersonal skills Ability to work effectively in a team setting Degree in social work or related field is preferred This position offers an opportunity to make a meaningful impact on the lives of individuals and families in need.
