JOB TITLE

$38•40 (depending on experience)
Shift:
Monday-Friday or Tuesday-Saturday (9:30pm•6pm) Chautauqua Nursing and Rehabilitation is seeking a dedicated Registered Nurse (RN)•Admissions to manage the admissions process for new residents and provide exceptional nursing care. This role is perfect for an RN with excellent communication skills and a passion for delivering high-quality care.
Key Responsibilities:
+ Coordinate the admission process for new residents, including assessments and documentation. + Conduct resident assessments and collaborate with interdisciplinary teams to develop care plans. + Ensure all admissions meet facility policies, state, and federal regulations. + Provide education to families and residents regarding services and procedures. + Act as a liaison between families, medical staff, and the administrative team to ensure smooth transitions. + Maintain accurate and up-to-date resident records in compliance with regulatory standards.
Qualifications:
+ Current and active Registered Nurse (RN) license in New York State. + Previous experience in admissions, case management, or a similar nursing role preferred. + Strong organizational, communication, and interpersonal skills. + Ability to work independently, manage multiple priorities, and ensure excellent patient care. + Compassionate, team-oriented, and able to build strong relationships with residents and families.

QUALIFICATIONS
EDUCATION
Preferred:
Associates Degree
RNEXPERIENCE
Minimum:
1 year of experience working in a long-term care setting working with residents and families
LICENSURE/CERTIFICATION
