Shift:
Monday-Friday or Tuesday-Saturday (9:30pm•6pm) Chautauqua Nursing and Rehabilitation is seeking a dedicated Registered Nurse (RN)•Admissions to manage the admissions process for new residents and provide exceptional nursing care. This role is perfect for an RN with excellent communication skills and a passion for delivering high-quality care.
Key Responsibilities:
Coordinate the admission process for new residents, including assessments and documentation. Conduct resident assessments and collaborate with interdisciplinary teams to develop care plans. Ensure all admissions meet facility policies, state, and federal regulations. Provide education to families and residents regarding services and procedures. Act as a liaison between families, medical staff, and the administrative team to ensure smooth transitions. Maintain accurate and up-to-date resident records in compliance with regulatory standards.
Qualifications:
Current and active Registered Nurse (RN) license in New York State. Previous experience in admissions, case management, or a similar nursing role preferred. Strong organizational, communication, and interpersonal skills. Ability to work independently, manage multiple priorities, and ensure excellent patient care. Compassionate, team-oriented, and able to build strong relationships with residents and families.

RN EXPERIENCE
Minimum:
1 year of experience working in a long-term care setting working with residents and families
LICENSURE/CERTIFICATION
