Instead of seeing a handful of patients each day, your work mayaffect millions for years to come. The primary role of the HomeInfusion Nurse is to provide clinical care and education tohome-based patients receiving a wide range of parenteralmedications dispensed from Optum Infusion Pharmacy (the “Company”).The most common route of administration is intravenous; howevermedications may also be given via subcutaneous infusion orinjection, intramuscular injection, or intraspinal (intrathecal andepidural) infusion. Service delivery takes place in patient homesand also in ambulatory infusion suites, where available. Allinfusion services and medication delivery is provided in accordancewith Optum Infusion Pharmacy policies, procedures andtherapy-specific protocols, and based on comprehensive ordersreceived from a physician or licensed practitioner. The HomeInfusion Nurse fills an integral role in the clinical operations ofthe organization, working collaboratively with themulti-disciplinary team needed to prepare, deliver and seekreimbursement for the infusion therapy that is safely andeffectively administered to each patient. Primary Responsibilities
- Establish and maintain positive clinical rapport with patientsand their families, working closely with them in the provision ofinfusion therapy, education related to self-care andself-administration, and clinical monitoring of patient response totherapy
- Assess patient appropriateness for home infusion therapy,including safety of the home environment, availability of caregiversupport, and willingness of patient and/or caregivers to become asindependent in therapy administration and monitoring of response totherapy as possible
- Obtain and document patient’s medical historyat the start of care and ongoing, particularly as related to thediagnosis being treated with the prescribed infusion therapy
- Complete all required clinical documentation thoroughly and timely,including nursing plans of care (POC), plans of treatment (POT),visit notes, clinical summaries, medication profiles, dischargesummaries and any updates provided to members of the patient’s careteam, as well as obtaining patient and/or caregiver signatures onrelevant consents and required forms
- Participate in caseconferences as needed to comprehensively plan patient care with themulti-disciplinary care team
- Provide skilled care to patients ofvarying ages and medical conditions, as outlined in the plan ofcare and in accordance with physician orders and the plan oftreatment
- Establish, secure and maintain vascular access devicesas prescribed, including (but not limited to) peripheral IV (PIV)placement, PICC and tunneled central venous catheter dressingchanges, and implanted port access
- Administer medications asprescribed, performing therapy-specific clinical assessments inaccordance with the Company’s policies, procedures and protocols,which may include frequent vital sign monitoring, management ofacute infusion reactions, monitoring of side effects, andprocurement of laboratory specimens
- Communicate observations ofpatient response to therapy and overall physicalassessment/condition with members of the care team in a timelymanner, which may include the nursing supervisor, dispensingpharmacist, registered dietitian, and licensed prescriber, amongothers
- Establish collaborative relationships with home healthagencies (HHAs) delivering services on behalf of/or in conjunctionwith the Company, providing agency education and support asrequested by the nursing supervisor
- Maintain a comprehensiveknowledge base of infusion therapies administered in the home andambulatory infusion center, utilizing Company policies, proceduresand protocols in addition to all available training resources toensure understanding of each therapy before initiating treatment
- Maintain current nursing licensure in all states in which patientsare serviced, as required by the respective State Boards of Nursingand/or Department of Health
- Comply with all State and Federalregulations and laws, including employee health and safetyprotections mandated by the Occupational Safety and HealthAdministration (OSHA), patient privacy and data security requiredby the Health Insurance Portability and Accountability Act (HIPAA),as well as all home infusion standards of practice
- Complete alltraining required upon hire and annually per policy, includingcardiopulmonary resuscitation (CPR) certification
- Serve as aprofessional representative of the Company when meeting with and/orproviding education to external customers, payers and referralsources
- Assist in staff orientation and training, serving as aclinical educator and resource as requested
- Drive personal/rentalvehicle (per travel policy) in the completion of job duties,including (but not limited to) travel to the Company office,patient homes, area hospitals, physician offices, home healthagency offices, and the Company’s Ambulatory Infusion Suite (AIS)You’ll be rewarded and recognized for your performance in anenvironment that will challenge you and give you clear direction onwhat it takes to succeed in your role as well as providedevelopment for other roles you may be interested in.
RequiredQualifications:
- Active and unrestricted licensure as a RegisteredNurse (RN) in the state of MN and/or SD
- Willing and able toobtain additional state licensures, if necessary
- Current BasicLife Support (BLS) and/or CPR certification, or willing to obtainto starting employment
- 1 years of experience working as an RN ina clinical care setting that contributes to proficiency in infusiontherapy (such as medical surgical unit, emergency department,intensive care unit, post-anesthesia care unit, or similar setting
- Proficient with electronic medical record documentation
- Willingand able to work with an interdisciplinary team of professionalsincluding pharmacists and technicians, physicians and otherlicensed prescribers, registered dietitians, home health agencies(HHAs), sales representatives, and others as needed to providecomprehensive care and support of patients and caregivers
- Willingand able to work independently in home or alternate-site settings
- Willing and able to assume a flexible work schedule
- Willing andable to meet attendance, overtime, after-hours on-call, and otherreliability requirements of the position
- Access to reliabletransportation that will enable travel to customer and/or patientsites
Preferred Qualifications:
- Experience administering infusiontherapy in a home or alternate-site setting
- Knowledge of andexperience with Microsoft Office and electronic medical records(EMR) programs
- Knowledge of the Infusion Nursing Society (INS)standards of practice, policies and procedures
- Knowledgeable ofHIPAA patient privacy and data security requirements, OSHA employeeprotection regulations, and accreditation standards for home healthand home infusion pharmacy (or other relevant accreditingorganizations)
- Proficient with Microsoft Office
- Provenexcellent organizational and critical thinking skills
- Proveneffective interpersonal communications, both written and verbal
- Proven effective teaching skills The hourly range for this role is$28.
61 to $56.06 per hour based on full-time employment. Pay isbased on several factors including but not limited to local labormarkets, education, work experience, certifications, etc.
