You will coordinate all aspects of medical care for consumers in their residences, ensuring their well-being and supporting their journey towards greater independence. If you’re a compassionate and resilient Registered Nurse with experience in mental health, behavioral health, adult, or geriatric care, and are committed to working with a vulnerable population, this role offers a unique opportunity to make a direct impact on the lives of individuals and their support teams.
What You’ll Do:
Coordinate Comprehensive Medical Care Coordinate all medical care for approximately 14 consumers across assigned residences. Serve as a clinician/coordinator of medical, physical, and sensory-motor care, with follow-up on clients’ care in audiology, dentistry, neurology, ophthalmology, physical examination and lab work, physical therapy, and psychiatry. Understand clients’ responses and behaviors as they relate to medications and clinical interventions. Provide the facility and staff with means for prompt detection and referral of health problems through adequate medical surveillance, periodic inspection, and regular medical examinations. Review lab work and intervene as needed based on findings. Monitor clients’ clinic appointments and doctors’ findings, overseeing all hospitalizations and emergency room visits. Monitor vital signs, blood levels, menstruation periods, hepatitis, tuberculosis, and seizure status. Prepare emergency medical kits for each residence according to American Red Cross guidelines and train staff in their use. Observe, report, and record pertinent client data, entering required nursing notes for monthly progress reports. Work with nutritionists in checking and re-checking client weights. Oversee Medication Management & Staff Training Periodically re-certify staff who are Approved Medication Administration Personnel (AMAP) . Supervise and review blood pressures, menstrual cycle charts, medication administration, hepatitis and PPD status, pharmaceutical interventions, first aid measures, prevention and control of communicable diseases, and client hygiene. Transcribe or supervise the transcription of doctors’ orders onto medication sheets. Review clients’ medications for potential adverse reactions, allergies, interactions, and contra-indications. Be responsible for the supervision of client medication administration. Report to the physician clients’ responses to medications and make recommendations accordingly. In consultation with the pharmacist, dispose of all medication not given to clients (excess, tainted, or misprescribed). Train staff in detecting signs of illness or dysfunction warranting medical or nursing intervention. Train staff in basic skills required to meet clients’ health needs and problems, and in first aid for accidents or illnesses. Plan and Liaise for Optimal Client Outcomes Serve as a clinical planner, reporting directly to the Nursing Coordinator regarding State regulation requirements for compliance. Make decisions with the Nursing Coordinator for presentation to the Inter-disciplinary Treatment Team to ensure compliance with medical/nursing care regulations. Possess knowledge of Medicaid standards as they relate to nursing interventions in the IRA. Write an annual Nursing Summary Evaluation and Service Plan for each client. Participate in the continuing interdisciplinary evaluation of each client to initiate, monitor, and follow up on their individualized medical needs. Attend and participate in staff conferences and Inter-disciplinary Treatment Team meetings, including 90-day and annual reviews. Report communicable diseases and infections in accordance with the law. Participate in pre-admission planning and evaluations, program designs, and client placement at the time of admissions. Coordinate annual physical examinations and monitor follow-up treatment. Liaise between residences and clinics, hospitals, and specialists for annual evaluations and follow-up appointments. Offer clients, as needed, health education, personal care/hygiene, and sex education.

Monday-Friday, 9:00 AM
- 5:00
PM On-Call:
24/5 Monday-Friday via phone (compensation built into salary). No weekend on-call (coordinators cover weekends).

Who You Are:
Credentials & Qualifications Registered Nurse (RN) New York State License. Minimum of 2 years of experience in Mental Health/Behavioral Health/Adult or Geriatric Acute care, or OPWDD experience required. (No women’s health or pediatrics experience accepted). Basic Life Support (BLS) certification. Strong organizational, communication, and interpersonal skills. Ability to work effectively with individuals with developmental disabilities, including those who may exhibit aggressive or violent behaviors (agency provides specialized training; employees assume inherent risks). Compassionate, patient, and able to handle challenging behaviors.
