Theterritory will be Queens NY. RN Must have a Car and DL in goodstanding to be considered. In this role, the RN will coordinate,implement and continually evaluate the home care needs of yourpatients. You will also review patient insurance information,coordinate activities involved in each individual care plan andmake sure that pertinent findings are shared with family andprofessional caregivers. Assess the patient for the appropriatelevel of care and utilization of agency resources. Perform acomprehensive physical, psychosocial and safety assessment viaexamination, observation and motivational interviewing. Medicationreconciliation Verify the initial plan of care with the patient’sphysician; utilizing patient specific parameters inform thephysician of any changes in the patients’ status; update the planof care to reflect changes made by the physician.
Ensure the EMRaccurately reflects:
the current plan of care; the servicesprovided; patient’s response to treatment; status of goalachievement. Accurately complete all OASIS data sets, within agencyestablished guidelines, which reflect positive clinical outcomesand consistent utilization of industry established best practices.
