This full time role hascoverage 4 days per week in Brooklyn where you will see patients inthe SNF, Rehab and community settings. The fifth day will be onsitein Manhattan at a SNF only doing Palliative Care consults. NPshould have at least 2 years experience in either Hospice,Palliative Care or Geriatrics to be considered. The NursePractitioner will provide direct skilled nursing care to patientsin the nursing home, hospital or community setting and will managepain and symptom management, communication including goal settingdiscussions, and hospice eligibility review. NP will do patientassessment – both cognitive and physical, and may be followed byone or more visits, as appropriate to the referral questions andstatus of the patient and family. NP will be responsible for takingpatient history, do appropriate examination / patient assessment,create and implement a plan of care to prevent or mitigate illnessand not burden for the patient and family. Orders appropriatediagnostic testing Formulates a problem list Recommendsinterventions consistent with a care plan that advances theobjectives of palliative care Provides interventions asappropriate, consistent with a care plan that advances theobjectives of palliative care If appropriate, takes the lead inimplementing key elements of palliative care, including familymeetings, goal setting discussions, and symptom management.
Performs hospice eligibility review and engages in accuratehospice-related discussions Collaborates with patients’ physiciansand other treatment team members Evaluates patient’s response totreatments, modifies or changes when needed.
