- Managed Long Term Care (MLTC)
- $20,000 sign-on bonus ($5,000 payable every six months, less taxes)

As an MLTC-Intake RN, you will assess potential new members for eligibility and explain the program to them and their families. You will play a crucial role in ensuring a seamless transition to managed long-term care.
Key Responsibilities:
Conduct outreach activities within the community to assess the eligibility and safety of potential members. Collaborate with the Intake/Outreach team and clinical team to ensure appropriate services are provided to new members. Communicate effectively with potential members, families, referral sources, and physicians.
Qualifications:
Current New York State Registered Nurse License. Bachelor’s degree in Nursing preferred. One (1) year of experience in an acute care setting. Must have UAS-NY experience. Home care and management experience preferred. Proficient in MS Office. Strong oral, written, and interpersonal communication skills. Bilingual in Chinese or Spanish required.
Compensation:
Annual salary: $95,000
