• Consultant will complete the UAS assessment remote.
• Consultant will review and fill out plans’ required forms at each assessment, and upload data and all documents to Guiding Care System, including: o Visit Acknowledgement Form o Informal Support Form o Caregiver And Doctor Information o Designated Representative Form o HIPAA Release Form o Enrollment Agreement o Service Consent o Home Safety Assessment o Home Visit Summary o PAH Education Provided o CDPAS Education & Forms – designated representative for CDPAS will need to have responsibilities reviewed at the assessment meeting •In a case where they did everything to get the docs but were still unsuccessful they should comment on that checklist § CDPAS Eligibility Form (for members receiving CDPAS services only) o All PCP/Specialist/Caregiver/Emergency contact information, including applicable NPI information o Time Tasking Tool (TTT) • Consultant will use QAS Analyzer for all assessments and provide evidence of outcomes o Once given the go-ahead, Consultant will sign and finalize the UAS within 24 hours for the MAP plan and within 48 hours for the MLTC o UAS assessment must be finalized on the same business day that they are completed or within 24 hours from assessment • Consultant is responsible to ensure that all documentation is securely transmitted to the Plan with UAS finalized within 24 hours of assessment for the MAP and 48 hours for the MLTC, as per NYS compliance regulations • Consultant will complete the Tasking Tool and upload documentation into Guiding Care o Upload TTT • Once visit is completed, Consultant sends activity to
