Job Description:

Position Overview :

The Registered Nurse – Home Health isresponsible for providing and documenting skilled nursing care inaccordance with the developed care plan and physicians’ orders foreach individual patient while adhering to confidentiality standardsand professional boundaries at all times.

Schedule:

Full-Time, 3days a week. Saturday & Sunday, plus choice of third day beingeither Friday or Monday.

Territory:

All communities within a 30mile radius of Rochester.

Essential Job Functions:

Develop a planof care through physician orders, client input, and nursingassessment to include medical interventions and measurable goalsand outcomes. Educate clients and their family members based onclient’s specific needs. Properly orient and train primarycaregivers to ensure the most optimal functioning level for eachclient. Coordinate the continuum of hands on client care throughdocumentation and timely communication with the client’s physicianand other caregivers. Provide care in patients home using a varietyof skills such as phone triage, patient education,observation/assessment, wound care, infusions, catheter care, PICCline dressing changes etc.

Anactive RN License in the state of application

Valid CPR Preferred:
