Fairport Rehab and Nursing Center Now Hiring:

Part Time RN Case Manager – Assisted Living/Adult Care Facility Join the dedicated team at Fairport Rehab and Nursing Center , a 142-bed Rehabilitation and Skilled Nursing Facility with over a century of service excellence. We offer a unique, person-centered approach to care by combining Assisted Living and Long-Term Care services within one integrated community. We are currently seeking a Registered Nurse (RN) Case Manager to support our Assisted Living/Adult Care Facility. This role is essential in ensuring the highest quality of care, promoting resident independence, and coordinating vital services.

Key Responsibilities:

Provide case management services to promote residents’ independence and choice. Orient new residents and families to the facility, routines, and available services. Act as a liaison between residents, families, healthcare providers, and community agencies. Organize and coordinate transportation for resident appointments. Perform medication audits, review weekly medication orders, and ensure PCP order renewals. Coordinate services with public/private sources including health, mental health, and social service programs. Assist residents with applications for public benefits and income entitlements. Monitor and respond to changes in residents’ condition; provide hands-on care when needed. Support and lead HHAs and PCAs, delegate tasks, and ensure adherence to safety protocols. Complete UAS assessments every 6 months (at minimum once every 12 months). Maintain professional collaboration with contracted CHHA services. Participate in the on-call rotation as needed. Perform additional duties as assigned by the Assisted Living Administrator.

Requirements:

Current New York State RN license in good standing.

Preferred Qualifications:

Geriatric nursing experience preferred. Knowledge of Adult Care Facility (ACF) and Assisted Living Program (ALP) regulations. Strong organizational and leadership skills. Familiarity with relevant regulatory agencies. Availability for holidays, weekends, and on-call responsibilities . Proficiency in reading, writing, and communicating effectively in English. Ability to perform essential job functions (reasonable accommodations considered). Flexibility, integrity, and the ability to work independently and collaboratively with a multidisciplinary team.
