Salary Range:

$71,612.39 – $110,999.20 The Case Manager isaccountable to facilitate the interdisciplinary team to plan,coordinate, implement and evaluate patient care for assignedservice line across the continuum of care. The Case Manager worksproactively with the Quality Improvement Teams, patient carestandards, Social Work, and utilization management to coordinatethe appropriate use of resources to achieve maximum clinical andfinancial outcomes. The Case Manager participates in maintainingquality care and performance improvement through leadership,problem solving, decision making, and outcome measurement. The CaseManager functions as a resource for the health care team,community, patient/significant others/family and payers byfunctioning as a clinician, consultant, advocate and educator forassigned service. . Coordination of Care a. Assists the admissionMD and or designated physician and the interdisciplinary teams inassuring coordination of care across the continuum of care in thehospital pre and post-op. b. Proactively monitor patients’ clinicalprocess through /patient care standards and evidence basedguidelines to ensure timely, appropriate interventions that achieveoptimal patient outcomes within appropriate LOS and financialconstraints. c. Provides collaborative care management with theprimary nurse in assessing for discharge planning needs,coordinating appropriate resources and evaluating effectiveness ofthe discharge plan. The discharge planning process needs to beginon admission. d. Collaborates with the health care team andappropriate department in the management of care across thecontinuum of care, including pre-admission, discharge,post-discharge, planning length of stay, and utilization ofresources. e. Utilizes own special body of knowledge and evidencebased guidelines to provide leadership and guidance to the healthcare team in formulating an individualized multidisciplinary planof care to include: pre hospitalization, acute hospital care,discharge education, transition to home and use of communityresources. f. Facilitates and participates in health care team careconference for patients with complex problems. g. Facilitatespatient and family education and the discharge process to promotecontinuity of care and optimal patient outcomes. h. Demonstratesexperience in the referral process and use of community resources.

Quality Improvement a. Collaborates with the health care team inimplementing strategies to reduce length of stay/resourceconsumption to optimize patient health status for an assignedservice patient. b. Assesses educational needs and provideslearning opportunities for health care professionals relevant toparticular cases and selected patient care groups. c. Demonstratescreativity and critical thinking ability in the development ofprograms or projects. d. Works with quality improvement team forcontinuous improvement of patient care outcomes while increasingquality and decreasing cost. e. Ensure that communication andtransmission of documentation occurs with community agencies.

Measurement a. Collaborates with case management leadership tocompile and report aggregate variances and data for specificpatient care services. b. Communicates and analyzes aggregatevariances with members of the health care team and developsstrategies for variance reduction. Thank you for your interest inAlbany Medical Center!?
