Compensation varies based on factors including, but not limited to,experience, skills, education, performance and salary range at thetime of the offer. Responsibilities Coordinate the overallinterdisciplinary plan of care for patient, from admission todischarge. Create plan for care across the continuum, integratingpatient/family preferences and values. Monitor patient care throughassessments, evaluations and/or patient records. Advocate forresources and removal of barriers. Maintain ongoing dialog withsupervisor and other health providers to ensure effectiveimplementation of health plan. Act as a resource for adequatemedical record documentation, appropriateness of services as theyrelate to diagnoses, and treatment options for post-discharge careRequirements Licensure / Certification /

Registration:

RegisteredNurse credentialed from the Wisconsin Board of Nursing obtainedprior to hire date or job transfer date required. Licensure fromthe Wisconsin Board of Nursing OR current home state license ifconsidered multi-state/Compact State. BLS Provider preferred.

American Heart Association or American Red Cross accepted. CaseManager credentialed from the Commission for Case ManagerCertification (CCMC) preferred.

Education:

Diploma from anaccredited school/college of nursing OR Required professionallicensure at time of hire.
