Are you a compassionate and skilled Registered Nurse (RN) looking to deliver care on your own terms—while supporting individuals in need of long-term services? 1st Choice is seeking Nurse Assessors/Case Managers to join the Independent Assessor (IA) Project, conducting both in-home and telehealth assessments for consumers applying for community-based long-term care programs. Key Responsibilities:
Conduct assessments (telehealth or in-home) using the Uniform Assessment System of New York (UAS-NY) to evaluate consumers’ health, functional abilities, and long-term care needs. Review medical documentation and referrals to ensure accurate, informed evaluations. Maintain detailed electronic documentation and accurately code UAS-NY data. Educate consumers on available healthcare options, managed care plans, and local community resources. Collaborate with interdisciplinary teams and the Clinical Quality Assurance Department to ensure compliance and quality care. Manage your own scheduling and appointment-setting to foster autonomy and balance. What You Need to
Succeed:
Education & Licensure:
Graduate of an accredited nursing program (BSN preferred). Active RN license in the State of New York. Valid New York State ID or Driver’s License.
Experience:
1-2 years of in-home care or community-based nursing experience preferred. Geriatric, home health, or chronic illness care experience is a plus. Familiarity with UAS-NY assessments and electronic documentation systems is highly desirable.
Skills & Requirements:
Strong interpersonal skills with cultural sensitivity and the ability to connect with diverse populations. Highly organized and self-motivated; capable of independently managing schedules and caseloads. Proficiency in electronic health records, telehealth platforms, and UAS-NY systems.
