Required:

NYS RN License, BLS, ACLS, Tele, Ventilator, 2years Tele ; Med Surg experience, Min 2 years RN experience, 1 yeartravel exp for Travel positions, EPIC experience highly preferred

Duties:

Registered Professional Nurse who provides safe, competentquality care based on nursing theory and research to a designatedgroup of patients and significant others.

Essential Duties andResponsibilities:
PATIENT CARE 1.

Delivers patient care byincorporating the tenets of the professional practice model ofRelationship Centered Care and through the application of thenursing process based upon theory, research, evidence-basedpractice, and approved organizational nursing standards. 2.Assesses and evaluates patient care needs and applies criticalthinking skills in patient care management. 3. Integrates relevantassessment and intervention skills in the delivery of nursing care.4. Collaborates with the interdisciplinary healthcare team in thedevelopment, implementation, and evaluation of the plan of care. 5.Prioritizes all aspects of patient care including teaching,rounding, coaching, and planning “after hospital care” anddelegating to others as appropriate. 6. Educates patients andcaregivers while anticipating needs and readiness to learn, abouttheir plan of care, transition of care, promotion of health, andprevention of disease. 7. Communicates effectively andprofessionally with patients, family and all members of theInterdisciplinary Patient Care Team. 8. Manages assignments withinthe Care Delivery Model of Modified Primary Nursing anddemonstrates allocation of material resources effectively.

PATIENTEXPERIENCE 1.

Role models the tenets of Mount Sinai RelationshipCentered Care (MSHRCC) through facilitation of the followingactions including, but not limited to, Admission Welcome, Bedsideshift Report, HELP (High Risk Medications, Equipment,Lines/Drains/Airway, Pain and Plan of Care) AM/PM Care, 5 MinuteSit Down, Purposeful Hourly Rounding, Medication Review, DischargeWrap Up, and MD/RN Unit Collaboration. 2. Considers the patient’svalues, preferences, cultural diversity, expressed needs andknowledge in all aspects of care. 3. Establishes and maintains atherapeutic relationship with the patient and family. 4. Usesevidence-based practices to increase understanding of patients’perceptions of care. 5. Initiates service recovery for patients andfamily members who have concerns and escalates to Clinical NurseManager/Assistant Nurse Manager/Nursing Shift Manager or PatientRelations as needed. 6. Demonstrates caring, respect, compassion,empathy and active listening through dialogue, body language andactions. 7. Supports staff use of evidence-based practices toincrease understanding of patients’ perceptions of care. 8.Conducts Purposeful Hourly Rounding (PHR) on assigned patientsaddressing the 4 Ps.

QUALITY AND SAFETY 1.

Practices safe andefficient patient-centered care, identifying and escalatingbarriers and opportunities for improvement. 2. Demonstratesclinical skills and knowledge crucial to quality and safety in thepatient population served. 3. Implements process improvementstrategies based on institutional, evidence-based ensure andprocedures. 4. Engages in formal and informal peer andinstitutional review processes. 5. Collaborates withinterdisciplinary teams to create and implement quality improvementprojects, evidence-based practice activities and nursing researchstudies. 6. Contributes to quality and safety practices andrequired compliance measures. 7. Implements principles of highreliability to identify and avoid high risk behaviors to provide asafe environment for patients. 8. Escalates potential safetyhazards or gaps from best practice following institutional chain ofcommand protocol. 9. Utilizes institutional information technologyby documenting nursing practice to support quality and performanceimprovement initiatives.

Skills:
OPERATIONS 1.

Performs chargenurse duties as assigned. 2. Coordinates with the ClinicalCoordinator/Clinical Nurse Manager/Assistant Nurse Manager/NursingShift Manager to ensure supplies are appropriately utilized andinventory is sufficient for patient care. 3. Escalates material orstaffing resource deficits to charge nurse, Clinical NurseManager/Assistant Nurse Manager or Nursing Shift Manager. 4.Demonstrates patient focused and cost effective approaches topatient care in terms of equipment, staff, supplies and all otherresources. 5. Documents in an accurate and thorough manner incompliance with hospital, regulatory and legal requirements andstandards of care. 6. Facilitates patient throughput on shiftbasis.

PROFESSIONAL DEVELOPMENT 1.

Contributes to the environmentof care to support clinical colleagues, patients and their familiesand members of the healthcare team. 2. Advances clinical competencein nursing practice to progress from novice to expert. 3. Projectsa professional image to colleagues and communicates with styles andmethods that demonstrate caring, respect, compassion and empathy.4. Incorporates ethical principles into decision making for patientand family. 5. Encourages and demonstrates a spirit of scholarship,inquiry, life-long learning and innovation for self and others. 6.Acquires knowledge and skills relative to the role, patientpopulation, clinical specialty and local and/or global healthcommunity needs. 7.
