Nursing Full Job Description Responsibilities:

Conduct comprehensive Start of Care assessments to develop personalized care plans tailored to individual client needs. Perform Revisit assessments to monitor and update care plans, ensuring the provision of effective and appropriate care. Conduct Post Hospitalization visits, coordinating with medical professionals and caregivers to facilitate smooth transitions from hospital to home. Provide supervision and training to Home Health Aides (HHAs) to ensure they deliver care in accordance with established standards and guidelines. Monitor client progress, address any concerns, and collaborate with the care team to make necessary adjustments to the care plan. Maintain accurate and detailed documentation of all assessments, visits, and client interactions. Collaborate effectively with physicians, therapists, and other healthcare professionals to provide holistic and coordinated care. Ensure compliance with all relevant regulations, policies, and procedures.

Qualifications:

Valid RN license in the state of New York. Minimum of 1 year of experience in home care nursing. Experience in conducting Start of Care assessments, Revisit assessments, and Post Hospitalization visits a plus Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Ability to work independently and as part of a team. Reliable transportation and willingness to travel within the Long Island area. Knowledge of Medicare and Medicaid regulations (preferred)
