The Senior Community Care (SCC) program is alongitudinal, integrated care delivery program that coordinates thedelivery and provision of clinical care of members in a facilitysetting. The APC within SCC provides care to our highest-riskhealth plan members and is part of an interdisciplinary team. Inthis role you will have the ability to achieve work life balance.

No on-call, no weekends, and no holidays required. This positionrequires travel through the state of XX. Primary Responsibilities

  • Provide clinical coverage for the primary APC during periods ofPTO or short/long term absences
  • Manage members through an acutemedical situation or an exacerbation of a chronic condition,including testing, diagnostics, and interventions
  • Manage membersduring a short term, sub-acute rehabilitation episode
  • Conductcomprehensive, monthly/routine, and/or acute care visit assessments
  • Effectively manage acute and chronic medical and behavioralconditions
  • Ensure accurate and complete documentation, includingICD 10 conditions
  • Communicate and collaborate with theinterdisciplinary care team You’ll be rewarded and recognized foryour performance in an environment that will challenge you and giveyou clear direction on what it takes to succeed in your role aswell as provide development for other roles you may be interestedin.
Required Qualifications:
  • Certified APC through a nationalboard
  • For NPs:

    Graduate of an accredited master’s degree inNursing (MSN) program and board certified through the AmericanAcademy of Nurse Practitioners (AANP) or the American NursesCredentialing Center (ANCC), Adult-Gerontology Acute Care NursePractitioners (AG AC NP), Adult/Family or Gerontology NursePractitioners (ACNP), with preferred certification as ANP, FNP, orGNP

  • For PAs:

    Graduate of an accredited Physician Assistant degreeprogram and currently board certified by the National Commission onCertification of Physician Assistants (NCCPA)

  • Active andunrestricted APC license in the State of Connecticut
  • Current,active DEA licensure/prescriptive authority or ability to obtainpost-hire, per state regulations (unless prohibited in state ofpractice)
  • Ability to gain a collaborative practice agreement, ifapplicable in your state
  • Ability to complete physicalrequirements, including moving a 30-pound bag and ability toperform physical assessment
  • Access to reliable transportation
  • 1years of clinical experience in practice (long-term care settingpreferred)
Preferred Qualifications:
  • Experience in meeting themedical needs of patients with complex behavioral, social and/orfunctional needs
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills including the ability to documentmedical information with written and electronic medical records
PLEASE NOTE

Employees must be in an active regular status.

Employees must remain in role for a minimum of 12 months from thedate of hire /rehire/transfer. If an employee leaves Home andCommunity, the student loan repayments will cease. The employeemust remain in an Advanced Practice Clinician or Physician rolewithin Home and Community for 36 months to receive the full benefitof the student loan repayments. The salary range for this role is$89,800 to $176,700 annually based on full-time employment. Pay isbased on several factors including but not limited to local labormarkets, education, work experience, certifications, etc.
