- MedSurg/Telemetry provides comprehensivenursing care to patients in a medical-surgical (MedSurg) unit witha focus on telemetry monitoring.
These patients may have a widerange of conditions, including cardiovascular issues, respiratoryconditions, post-operative recovery, and other acute illnesses orinjuries that require constant monitoring. The RN in this role isresponsible for administering medications, monitoring vital signsand heart rhythms, performing assessments, collaborating withhealthcare teams, and providing patient education. The primary goalis to ensure the safety, comfort, and optimal recovery of patients./n/n
Key Responsibilities:

Patient Assessment andMonitoring:
/n /n
- Perform comprehensive and continuousassessments of patients, including evaluating vital signs, heartrhythms (via telemetry), and overall condition. /n
- Monitor andinterpret telemetry data to detect any irregularities in heartrate, rhythm, and other vital parameters, notifying the healthcareteam if needed. /n
- Assess physical, emotional, and psychologicalneeds of patients, developing individualized care plans to promoterecovery and wellness.

Medication Administration andManagement:
/n /n
- Administer prescribed medications, includingoral, intravenous, and intramuscular medications, while ensuringproper dosing and timing. /n
- Monitor patients for side effectsand adverse reactions to medications, immediately reporting anyissues to the physician or healthcare team. /n
- Provide painmanagement interventions and adjust care based on patient responseand comfort level.

Telemetry and Cardiac Monitoring:
/n /n
- Monitor patients with telemetry units to assess heart rate,rhythm, and overall cardiac health, documenting and reporting anyirregularities to the medical team. /n
- Assist in theinterpretation of telemetry readings, ensuring prompt interventionfor any abnormal findings such as arrhythmias, tachycardia, orbradycardia. /n
- Maintain and troubleshoot telemetry equipment toensure proper function and reliable patient monitoring.

Wound Care and Post-Surgical Care:
/n /n
- Providepost-operative care for surgical patients, including monitoringincision sites, managing drains, and ensuring proper wound healing./n
- Perform dressing changes, assess for signs of infection, andeducate patients on post-surgical care instructions. /n
- Administer treatments to manage or prevent complications, such asdeep vein thrombosis (DVT), pressure ulcers, or infections.

Patient Education:

- Educate patients and theirfamilies on disease management, including medication regimens,lifestyle modifications, and post-discharge care. /n
- Offerguidance on managing chronic conditions such as hypertension,diabetes, and heart disease, ensuring patients understand theirrole in maintaining health. /n
- Provide education on fallprevention, nutrition, activity restrictions, and dischargeplanning.

Healthcare Team:
/n /n
- Work closely with physicians, nurse practitioners, physicianassistants, and other healthcare providers to implement and adjustpatient care plans. /n
- Participate in daily rounds to discusspatient progress, care needs, and treatment adjustments. /n
- Collaborate with interdisciplinary teams, including physicaltherapy, social work, and dietitians, to provide holistic care topatients.

Emergency Response and Critical Care:
/n/n
- Respond promptly to medical emergencies, including cardiacarrests, respiratory distress, and sudden changes in patientcondition. /n
- Provide immediate interventions, including CPR,advanced cardiac life support (ACLS), or other life-savingmeasures, in collaboration with the healthcare team. /n
- Managepatients who experience acute changes in their condition,escalating care as necessary (e.
g., transferring to ICU ifrequired). /n /n /n 8. /n
Documentation and Reporting:

- Maintain accurate, timely, and thorough documentation of allassessments, interventions, patient responses, and communicationsin the electronic health record (EHR). /n
- Ensure that patientinformation is up-to-date and readily available for otherhealthcare professionals involved in patient care. /n
- Report anychanges in patient condition or concerns to the appropriatehealthcare provider and follow up on recommendations.

Infection Control and Safety:

- Follow infection controlprotocols to prevent the spread of infections, including properhand hygiene, use of personal protective equipment (PPE), andsterile techniques when necessary. /n
- Ensure patient safety byregularly assessing for fall risks, managing medications safely,and preventing hospital-acquired infections. /n
- Maintain a cleanand safe environment for patients by adhering to hospital policiesand infection prevention measures.

QualityImprovement and Continuing Education:

- Participate inhospital quality improvement initiatives and patient safetyprograms aimed at enhancing care delivery and outcomes. /n
- Staycurrent with the latest nursing practices, healthcare technologies,and patient care protocols, especially in telemetry monitoring andMedSurg nursing. /n
- Attend continuing education workshops,certification programs, and professional development opportunitiesto maintain licensure and expertise.
