Position in these functionsworks as a provider member within the HBMC interdisciplinary teamin the direct delivery of home-based medical care of a populationwithin an assigned geography. - being of patients, this position collaborates with the
PCP,HBMC
interdisciplinary teammates and consultants, and as needed,the health plan Medical Director and other health professionals, todevelop and carry out patients’ goals of care in all phases of thepatient journey.
Primary Responsibilities:
- Performs an initialcomprehensive assessment on all newly enrolled patients andprovides ongoing care thereafter
- Develops a patient managementcare plan upon enrollment and updates it as needed when changes incondition warrant or following hospitalization
- Meets withpatients and/or their legal representatives to review newlydeveloped or modified care plans; involves the PCP or supervisingphysician in these meetings, when applicable
- Comfortable withbasic procedures of nursing care, including IV placement, blooddraws, injections, foley catheters, nasal packing, wound care, etc.
New hire and annual skills check/training provided
- Prescribesappropriate diagnostics and interventions to avoid unnecessaryacute admissions
- Completes follow-up and post-dischargeassessments according to documented standard operating procedure
- Consults with hospital, emergency or post-acute clinical teamfollowing notification of patient transfer
- Educates patientsand/or their legal representatives in disease processes affectingpatients and ways to manage them effectively, as well as to promotewellness
- Actively participates in ongoing meetings pertaining topatient care and clinical excellence
- Implements HEDIS measurecampaigns and other quality initiatives to ensure the higheststandards of care and to promote the improvement of care managementand delivery
- Keeps current on relevant medical and nursingresearch, technology, and related issues by attending continuingeducation courses, professional meetings and journal reviews
- Practices in accordance with the respective state laws andregulations governing the practice of advanced practice nursing orphysician assistants You’ll be rewarded and recognized for yourperformance in an environment that will challenge you and give youclear direction on what it takes to succeed in your role as well asprovide development for other roles you may be interested in.
Required Qualifications:
- Completion of Nurse Practitioner ofPhysician Assistant accredited program
- Current state RN/NP or PAlicense
- Certification through the American Academy of NursePractitioners (AANP), American Nurses Credentialing Center (ANCC),or National Commission on Certification of Physician Assistants(NCCPA)
- Access to reliable transportation; if you are driving avehicle, you must comply with all the terms of the Optum MotorVehicle Safety policy
Preferred Qualifications:
- 2 years ofclinical experience, primary care, home-based medical care,geriatrics or palliative care of emergency/urgent care
- Knowledgeof long-term care, community resources and cost-effectivealternatives regarding patient care delivery systems
- Knowledge ofCPT, ICD-10 and HCPC codes
- Knowledge of clinical standards ofcare
- Awareness of NCQA requirements, CMS guidelines,Medicaid/Medicare contracts and benefit systems The salary rangefor this role is $104,500 to $156,000 annually based on full-timeemployment.
Salary Range is defined as total cash compensation attarget. The actual range and pay mix of base and bonus is variablebased upon experience and metric achievement. Pay is based onseveral factors including but not limited to local labor markets,education, work experience, certifications, etc. UnitedHealth Groupcomplies with all minimum wage laws as applicable. PLEASE NOTE
Employees must be inan active regular status. Employees must remain in role for aminimum of 12 months from the date of hire /rehire/transfer. If anemployee leaves Home and Community, the student loan repaymentswill cease. The employee must remain in an Advanced PracticeClinician or Physician role within Home and Community for 36 monthsto receive the full benefit of the student loan repayments.
