Job Summary Provides direct and indirect patient care in theoffice setting to patients, families and caregivers in the MedicalGroup practice. Uses the nursing process consistent with acceptablestandards of the profession and MVHS Medical Group. Provides highquality patient centered care that reflects initiative, flexibilityand responsibility with a minimum amount of administrativesupervision. Determines priorities of care based on physical andpsychosocial needs, as well as factors influencing patient flowthrough the system. Communicates with outpatient clinic physiciansand NP/PAs (healthcare providers) about changes in patient’sstatus, symptomatology and results of diagnostic studies. Is ableto respond quickly and accurately to changes in condition orresponse to treatment. Core Job Responsibilities
- Support the PCPin the delivery of care by gathering and documenting relevantclinical data, as appropriate and per patient need.
- Assists inspecialty office procedures as directed.
- Participate in the dailyhuddle to identify individual patient needs for upcomingappointments.
- Clinical liaison between patient and provider forquestions, concerns and testing results.
- Communicate effectivelywith other members of the care team in coordinating theindividualized plan of care.
- Support PCP during in officeprocedures and examinations.
- Provide outreach and education topatients in need of services as identified through qualityimprovement initiatives (Population Health Management).
- Performduties within scope of practice to the highest level of licensure.
- Provide direct support & guidance to other members of the careteam as appropriate.
- Able to perform assessment data collectionon all patients utilizing tools / forms in EMR.
- Adequately assessand reassess pain. Educate the patient and family regarding painmanagement.
- Provide age appropriate care to patients in thecommunity setting.
- Consistently document procedures completed inthe EMR, to include medications administered and follow-upassessments
- Deliver medication utilizing the five (5) rights andobserves patient for desired untoward effects.
- Maintain currentknowledge of medications and their correct administration.
- Establish priorities of patient care based on needs and officeresources.
- Demonstrates the ability to assist providers withprocedures and perform services requiring technical and manualskill
- Perform other duties as required.
Education/ExperienceRequirements Required:
- Graduate of an accredited school ofnursing or nursing program.
- Current New York State RegisteredNursing registration.
- BLS Certification.
- Minimum 3 years ofnursing experience
- Basic computer and EMR knowledge.
- Excellentcommunication and interpersonal skills.
- Ability to assimilateinformation from many different sources.
- Must be able tocommunicate in English both verbally and in writing.
Preferred:
Bachelor of Science in Nursing from accredited nursing program orhigher degree
