Field Home Care RN In-person Visits (Bilingual English/Cantonese or Mandarin) 3.5 3.5 out of 5 stars Queens, NY 11361 The RN Field Nurse provides high-quality nursing care to patients in a home or community setting. This role involves assessing, planning, implementing, and evaluating patient care while working independently and collaborating with other healthcare professionals. The RN Field Nurse ensures that patients receive appropriate medical care and support in their home environment. Key Responsibilities Perform comprehensive assessments of patients’ physical, emotional, and psychological needs. Develop individualized care plans in collaboration with patients, families, and healthcare providers. Provide guidance on medication management, lifestyle changes, and preventive care. Document all patient interactions, assessments, and care provided in a timely and accurate manner.
