The ideal candidate will provide high-quality patient care, focusing on preventive health measures and the management of chronic conditions. You will work collaboratively with physicians and other healthcare professionals to deliver comprehensive care to patients in various settings, including primary care, home care, and hospital environments. Responsibilities Conduct thorough patient assessments, including physical examinations and health histories. Develop and implement individualized care plans based on patient needs. Perform diagnostic tests and interpret results, including spinal taps when necessary. Provide education to patients and families about health management and disease prevention. Administer medications and treatments as prescribed, adhering to aseptic techniques. Collaborate with interdisciplinary teams to ensure continuity of care across various settings. Monitor patient progress and adjust treatment plans as needed. Maintain accurate medical records in accordance with standards and regulations. Stay updated on advancements in medical terminology and practices related to primary care, ICU, OR, hospital medicine, and dementia care. Qualifications Master’s degree in Nursing from an accredited program. Current Nurse Practitioner certification with a valid state license. Strong knowledge of medical terminology and aseptic techniques. Experience in primary care or relevant clinical settings preferred; experience in ICU or OR is a plus. Familiarity with home care practices and standards of dementia care is desirable. Excellent communication skills and the ability to work collaboratively within a healthcare team.
