M-F 9am-5:30pm About the Role We are seeking a compassionate and driven Registered Nurse (RN) to join our interdisciplinary care team in Queens, NY . In this community-based role, you will provide person-centered care and clinical excellence to patients in their homes or care facilities. Duties will include:

Practice independently in the community as a key member of an interdisciplinary care team. Deliver personalized nursing care and case management in patients’ homes or residential care settings. Evaluate patients’ evolving medical needs and adjust plans of care accordingly. Collaborate with other healthcare professionals to ensure high-quality, coordinated care.

Qualifications will include:

Current NYS RN License and registration in the State of New York (required) Minimum of 1 year nursing experience in a medical/surgical environment Valid driver’s license or NYS Non-Driver Photo ID may be required Strong clinical assessment and critical thinking skills Excellent communication and organizational abilities Wound Care Certification – Required or willingness to obtain within 1 year of hire Certification in Hospice and Palliative Care Nursing – Required #
