RN MDS accessor 3.5 3.5 out of 5 stars 140 Beach 114th Street, Rockaway Park, NY 11694 Job Overview The primary role of the RN MDS Assessment Coordinator is to assist in gathering of clinical documentation and completion of the non-comprehensive MDS assessments and ensures that essential information gathered during the assessment period, is integrated in the care planning process. Responsibilities Complete and oversee MDS assessments as directed by Director of MDS Ensure that the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process Educate the interdisciplinary team about accuracy of clinical documentation and its importance in all aspects of care delivery Coordinate the timely completion of the MDS assessments with the interdisciplinary team within the guidelines and specified time frames as per RAI Manual Assist in the review of the clinical record accuracy of MDS coding and completeness of documentation. Act as liaison to the interdisciplinary team in the discussion of essential findings of a resident’s condition obtained while completing MDS assessments Assist in transmission of MDS assessments, and obtains validation records and reviews them for needed corrections Other duties as assigned or as deem necessary and appropriate, as indicated by Director of MDS Skills Strong background in patient care , particularly in senior care and geriatrics . Proficient in patient assessment techniques for diverse populations Experience with Minimum Data Set (MDS) 3.0 assessments for long-term care facilities is advantageous. Excellent communication skills to effectively interact with patients, families, and healthcare team members. Ability to work collaboratively in fast-paced environments while maintaining attention to detail. Education Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program. Experience Experience in long-term care and experience in MDS. Must have familiarity with various computer software Must be efficient, organized, detailed -oriented and flexible Must have excellent time management and consistently meet task deadlines Join our team of dedicated professionals committed to making a difference in the lives of our patients.
