(including 100% of Hourly Wage Paid for Drive Time) BonusEligible We provide in-home care to former Nuclear Weapons Workerswho are suffering from chronic and terminal illnesses, as a resultof their previous work environment. Our Senior RN-Case ManagersDirect assigned team members of RN Case Managers in the provisionof care in accordance with Agency policy and with state-specificnurse practice act, and regulatory requirements. QualificationsGraduate of a state approved school of professional registerednursing BSN preferred Current, unrestricted RN license in thestate(s) of practice Minimum of two (2) years nursing experienceincluding one (1) year in home care or closely related field One(1) year of supervisory and/or case management experience preferredCurrent CPR certification Essential Functions/Areas ofAccountability Responsible for functions and accountabilities ascontained in the case manager job description Provide direct careand case management of assigned clients Assist and collaborate withthe regional director and other personnel to identify and correctissues and/or improve services. Plan, implement, and evaluate careprovided Participate, coordinate and manage client care conferencesas needed. Serve as a local on-site clinical resource as needed andprovides support to ensure client’s home care needs are met. Assistand collaborate with staffing coordinators regarding theappropriateness of staffing and scheduling of personnel withinscope of practice, competencies, client needs and complexity ofhome care. Adhere to nursing delegation guidelines as described inAgency Scope of Practice policy. Ensure adherence to Agencypolicies. Perform other functions as requested by the regionaldirector which may include the following: Participate ininterviewing, selection, and ongoing evaluation of clinicalpersonnel as requested by the Regional Director Personnel training,education, and competency validation Review and evaluate clinicaldocumentation for accuracy and completeness Participate in allAgency performance improvement initiatives including but notlimited to quarterly medical record review Collect, document, andsubmit data on infections, occurrences, complaints and grievances,and performance improvement activities Perform and documentsupervisory visits as indicated to facilitate problem resolutionReview nurse shift reports for adherence to policy and foropportunities for performance improvement Home chart completenessTimeliness of staffing cases post referral Equipment trackingAssist with marketing activities such as visiting with clients orphysicians to discuss Agency programs as requested The senior casemanager, or similarly qualified alternate, shall be available atall times during operating hours and participate in all activitiesrelevant to the professional services furnished, including thedevelopment of qualifications and the assignment of personnel.

Standard Rate:
