Role:

We are looking for a compassionate and highly organized Nurse Care Coordinator to join our team at a well-established nursing home in Staten Island. In this leadership role, you’ll be responsible for overseeing and coordinating comprehensive care plans to ensure our residents receive the highest quality of care and attention every step of the way.

What You’ll Do:

Develop, review, and update individualized care plans for residents in collaboration with interdisciplinary teams Serve as the liaison between nursing staff, physicians, families, and outside healthcare providers Monitor residents’ clinical status, ensuring that care plans reflect changes in condition Facilitate communication during resident transitions (admissions, discharges, transfers) Ensure care delivery meets DOH regulations and long-term care best practices Participate in quality assurance programs and audits Provide support to families and serve as a resource for resident advocacy Occasionally assist during weekend coverage as needed

Requirements:

Active NYS Registered Nurse (RN) license 2+ years of nursing experience in a long-term care or skilled nursing facility Prior experience in care planning, case management, or clinical coordination is strongly preferred Strong documentation, organizational, and communication skills A genuine passion for elderly care and team collaboration
