Nurse Case Manager Position Available In Richmond, New York

Job Summary Our client is seeking a Nurse Case Manager to join their Nurse Family Partnership (NFP) program, which is a nationally recognized, evidence-based nurse home-visiting program. The primary responsibilities include assessing the medical and social needs of pregnant women and infants, following a prescribed process/plan of care, and monitoring outcomes in accordance with NFP and client policies and procedures. Key Responsibilities Provide home visits to pregnant/parenting women and their families eligible for the NFP Program. Provide education and case management services from pregnancy through the child’s second birthday. Adhere to the nursing process and the NFP model of home visitation. Maintain a caseload of at least 25 clients. Clinical Duties Develop therapeutic relationships using reflection and motivational interviewing. Perform home visiting in accordance with the NFP model and guidelines. Assess needs related to NFP domains and assist families in goal setting. Provide education, support, and referrals to help families attain their goals. Consult and collaborate with other professionals and follow up on service delivery. Complete all required documentation for Medicaid billing. Qualifications BSN required or associates if currently enrolled in a BSN program. Current New York State RN license (in good standing) is required. Current CPR certification. Bilingual English/Spanish preferred. Home visiting experience preferred. Excellent written and verbal communication skills. Basic computer skills. Additional Requirements Valid driver’s license and insured automobile may be required. May require working a flexible schedule including some evenings and weekends.
