$60,028.00 – $99,023.00 RN forCardiothoracic division outpatient office within the department ofsurgery. Collaborative team effort to provide optimum care topatients requiring scheduled and emergent surgical care. Thisposition supports the thoracic team but also cross covers thecardiac team when required. Under the supervision of the NursingSupervisor and Nurse Manager, the Ambulatory Care RN providesnursing support to primary and subspecialty providers. This isdemonstrated by the use of the nursing process to workcollaboratively with the healthcare team in planning, directing,providing, and evaluating patient care. The Ambulatory Care RN isdefined within the department to provide patient care, triage,consultation, and referral among team members.

Support for Self-Management Enhances healthliteracy Performs a baseline comprehensive needs assessmentreflecting patient values, preferences, and goals (in person or viatelehealth). Analyzes patient care needs and utilizes a systematicapproach to the delivery of care. Provides age-appropriate directpatient care including family and patient. Advocacy Negotiates andsecures patient services; coaches the patient in self-advocacy Onthe phone and at the office visit, listen to the patient and familygoals and concerns. Works with patients, insurance companies, homecare agencies, etc. to access providers, community services,medications. With an initial order of medications, enteralformulas, – verifies if PA needed, or med not covered at all,reviewing other possible options for non-formulary med, clarifyingorders, renewing orders Education and Engagement of the patient andfamily Assess readiness to learn/learning styles. Provides patientand family education taking into consideration cultural, religious,language, and motivational, physical, emotional, and cognitivefactors into account in the teaching plan On the phone or in theoffice instructs about nursing care plans related to results oflabs, tests, procedures, medication changes, dietary changes, otherchanges in interventions. Provides education regardinginterventions and requests, “teach-back” to ensure understanding(in person and via telehealth). Performs telephone triage includingcoordination of care and documentation. Works with physicians andother resources to develop teaching tools for families and patientsand/or utilizes educational resources on Intranet. Cross settingcommunication and transition Develops the individualized care plan;care planning activities transcend barriers/transitions keeping thepatient at the focus. Nurses utilize evidenced-based practice toplan and implement nursing care plans (interventions and outcomes).The plan is updated with every phone call, office visit, inpatientadmission, etc. Coordinates and collaborates between specialty andprimary providers who develop and share the patient care planacross settings Care plan transmitted between setting, changes, andupdates communicated Discusses the discharge plan of care forpatients that are inpatient and coordinates follow-up interventionsneeded. Coordinates admissions with communication regarding theplan of care. Coordinates referral to the emergency room Takescalls by MD offices to discuss coordination of care that cannot beaddressed by faxing the most recent office visit note. Works withother departments inside and outside of AMC to coordinatemultidisciplinary care Coordinates home care – setting up forsupply deliveries, clarifying verbal orders Coordinates infusions-insurance, orders, communication to patient, education Suppliesinpatients with educational information as needed Coordinatesreferrals to social services departments, WIC Facilitates theutilization of resources to meet patient outcomes (casemanagement). Writes letters of medical necessity as indicatedParticipates in the patient throughput process in the clinic asindicated and needed (including vital signs, updating medicalrecords, performing diagnostic procedures, assisting the physicianand practitioners, facilitating teaching, end of appointmentcoordination of care) Coaching and counseling of patients andfamilies Answers questions patients have before and after providervisit. Patients are directed to bring questions to the visit and/orcalls and answers are secured, before, during, and after the visit(in person or via telehealth). Documents properly, givingage-appropriate care, gathering data correctly (including vitalsigns), and facilitating care in a customer-friendly manner.
Performs diagnostic tests such as impedance studies, wirelesscapsule endoscopies Nursing Process Assesses patient for knowledgeunderstanding diagnosis, needs, treatment, expected outcomes oftreatment. The best evidence is used for interventions/outcomes;the care plan is routinely updated. Teamwork and CollaborationInclusion of teamwork in orientation and continuingeducation-interdisciplinary. Enhanced utilization ofinterdisciplinary resources. FMLA paperwork, CPS paperwork, 504plans- coordination of care Patient-Centered Care planningMotivational interviewing; eliciting patient’s goals andpriorities. Individualized care plan; care planning activitiestranscend barriers/transitions keeping the patient at the focusUnderstands and supports their role in regulatory /complianceissues/the health care team. Maintain division medical recordsCompletes organized and quality documentation as required by thedepartment. Works with division physicians, administrative staff,and Nurse Manager to assure compliance with regulatory conditionsand policies and procedures Actively participates in the surveyprocess in regards to accreditation Maintains compliance withregulatory agencies Realizes their role in Environment of CareSurveys. Maintains a safe environment within the clinic and workingoffice space Maintains compliance with AMC standards for electronicsystem use Maintains compliance with AMC Code of ConductCollaborates with administrative staff and physicians on theQuality Improvement Team to address clinical and non-clinicalissues within the division Locates regulations and policies andprocedures online and know how to apply to current job Utilizeseducational resources on Intranet for self-education and educationof other staff Utilizes the Chain of Command as indicated Ensuresthat medical equipment functioning properly and knows the processto manage failure as the need arises Cross covers in otherdivisions within the department as needed Orientation of new staffDelegates daily tasks to appropriate clinical staff and ensurescompletion of tasks Reviews and triages incoming test results asindicated within the division. Thorough knowledge of organization, department,and/or section policies and practices related to position preferredAbility to administer medications via all routes and ability tocalculate medication doses. Proficiency in the operation ofstandard office equipment (Adding Machine, Fax, Photocopier, CreditCard Machine) and clinical equipment (Sphygmomanometer, ElectronicVital Signs machine, pulse oximetry machine, infant scale, adultscale, and stadiometer), Type 45-60 WPM on a Typewriter or ComputerWork Environment (including Hazards): A clinical care settingrequires the ability to lift a minimum of 10 pounds, extensivewalking, and standing. May be subjected to infectious materials andchemicals (i.e. HLD Solutions, disinfectant sprays, or wipes).
