Nurse Navigator – Oncology Position Available In Schenectady, New York

The RN Nurse Navigator coordinates aspects of care in collaboration with the multidisciplinary team for patients within specialized disease state to ensure they receive quality and comprehensive services. Serves as a consistent point of contact for physicians, patients, caregivers, families, and the community. Provides guidance to the healthcare team regarding patients in need of coordination of care. Assesses community needs and provides education, screening, support, referrals, coordination of care, and any other assistance identified as necessary. Ensures that patients and families are maintained at the highest level of functioning feasible. Works with patient, families, internal departments and external agencies to provide/coordinate smooth, timely care across the continuum. + Activities will be documented as per policy. Information will be analyzed, tracked, trended and shared with the appropriate medical staff, hospital department or community organizations as needed. Ensures that outcome management (data collection through analysis) information is accurate, communicated and evaluated for process improvements potential.

EDUCATION AND EXPERIENCE REQUIREMENTS

+ Graduate of accredited RN program. Current licensure and registration in NYS with Bachelors’ degree in healthcare related field. + Master’s Degree Preferred. + Valid NYS licensure as a Registered Professional Nurse. + The candidate must have at least five years of nursing experience, Oncology and/or case management experience preferred. + ONS/ONCC Oncology Nurse Certification or Case Management Certification preferred. Salary Range:

$36.07-54.10 /hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
