- MedSurg/Telemetryprovides comprehensive nursing care topatients in a medical-surgical (MedSurg) unit with a focus ontelemetry monitoring.
These patients may have a wide range ofconditions, including cardiovascular issues, respiratoryconditions, post-operative recovery, and other acute illnesses orinjuries that require constant monitoring. The RN in this role isresponsible for administering medications, monitoring vital signsand heart rhythms, performing assessments, collaborating withhealthcare teams, and providing patient education. The primary goalis to ensure the safety, comfort, and optimal recovery of patients.
Key Responsibilities:

Patient Assessment and Monitoring:
- Perform comprehensive and continuous assessments of patients,including evaluating vital signs, heart rhythms (via telemetry),and overall condition.
- Monitor and interpret telemetry data todetect any irregularities in heart rate, rhythm, and other vitalparameters, notifying the healthcare team if needed.
- Assessphysical, emotional, and psychological needs of patients,developing individualized care plans to promote recovery andwellness.

Medication Administration and Management:
- Administerprescribed medications, including oral, intravenous, andintramuscular medications, while ensuring proper dosing and timing
- Monitor patients for side effects and adverse reactions tomedications, immediately reporting any issues to the physician orhealthcare team.
- Provide pain management interventions and adjustcare based on patient response and comfort level.

Telemetry andCardiac Monitoring:
- Monitor patients with telemetry units toassess heart rate, rhythm, and overall cardiac health, documentingand reporting any irregularities to the medical team.
- Assist inthe interpretation of telemetry readings, ensuring promptintervention for any abnormal findings such as arrhythmias,tachycardia, or bradycardia.
- Maintain and troubleshoot telemetryequipment to ensure proper function and reliable patientmonitoring.

Wound Care and Post-Surgical Care:
- Providepost-operative care for surgical patients, including monitoringincision sites, managing drains, and ensuring proper wound healing
- Perform dressing changes, assess for signs of infection, andeducate patients on post-surgical care instructions.
- Administertreatments to manage or prevent complications, such as deep veinthrombosis (DVT), pressure ulcers, or infections.

PatientEducation:
- Educate patients and their families on diseasemanagement, including medication regimens, lifestyle modifications,and post-discharge care.
- Offer guidance on managing chronicconditions such as hypertension, diabetes, and heart disease,ensuring patients understand their role in maintaining health.
- Provide education on fall prevention, nutrition, activityrestrictions, and discharge planning.

Healthcare Team:
- Work closely with physicians, nursepractitioners, physician assistants, and other healthcare providersto implement and adjust patient care plans.
- Participate in dailyrounds to discuss patient progress, care needs, and treatmentadjustments.
- Collaborate with interdisciplinary teams, includingphysical therapy, social work, and dietitians, to provide holisticcare to patients.

Emergency Response and Critical Care:
- Respondpromptly to medical emergencies, including cardiac arrests,respiratory distress, and sudden changes in patient condition.
- Provide immediate interventions, including CPR, advanced cardiaclife support (ACLS), or other life-saving measures, incollaboration with the healthcare team.
- Manage patients whoexperience acute changes in their condition, escalating care asnecessary (e.g., transferring to ICU if required).

Documentationand Reporting:
- Maintain accurate, timely, and thoroughdocumentation of all assessments, interventions, patient responses,and communications in the electronic health record (EHR).
- Ensurethat patient information is up-to-date and readily available forother healthcare professionals involved in patient care.
- Reportany changes in patient condition or concerns to the appropriatehealthcare provider and follow up on recommendations.

InfectionControl and Safety:
- Follow infection control protocols to preventthe spread of infections, including proper hand hygiene, use ofpersonal protective equipment (PPE), and sterile techniques whennecessary.
- Ensure patient safety by regularly assessing for fallrisks, managing medications safely, and preventinghospital-acquired infections.
- Maintain a clean and safeenvironment for patients by adhering to hospital policies andinfection prevention measures.

- Participate in hospital quality improvementinitiatives and patient safety programs aimed at enhancing caredelivery and outcomes.
- Stay current with the latest nursingpractices, healthcare technologies, and patient care protocols,especially in telemetry monitoring and MedSurg nursing.
- Attendcontinuing education workshops, certification programs, andprofessional development opportunities to maintain licensure andexpertise.
