Job Description
1/2 REMOTE – HOME CARE RN – Suffolk County Burco Staffing LLC Suffolk County, NY We are looking for talented RNs with Home Care experience to conduct Assessments 1/2 REMOTE and 1/2 IN
THE FIELD.
Meaning that 2-3 days a week you would be conducting assessments via Televideo (3 assessments per day) and the other 2-3 days you would be completing F2F assessments in the
SUFFOLK COUNTY
area. (2 assessments per day) (Other Counties are also available.)
- This is a CRUCIAL role where you will be in a position to directly assist elderly consumers to qualify for Home Care and Long Term Managed Care services.

- Candidate MUST have at least 2 years of combined Home Care or care management experience

Responsibilities will require you to:
1. Perform initial In-home or Televideo evaluations of patients, that include health status, strengths, and care needs. 2. Review consumer medical documentation and or health referral forms, as relevant to the case 3. Enter evaluation data into an electronic form, to be transmitted as required 4. Document any concerns, conflicting information, other issues that surface during the evaluation process 5. Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan 6. Discuss with consumer health care options, supports needed, service vendor options and waiver options
Requirements:
Active NY RN License. Experience conducting UAS or OASIS assessments preferred. Excellent Patient Care and Assessment Skills Excellent verbal and written communication skills.’
