What You’ll Do:
Assume primary responsibility forpatient and family caseload, including the assessment, determiningeligibility, admission processing, planning, implementation, andevaluation phases of the nursing process Obtain data and assessnecessary information from patient physical, psychological, social,and spiritual factors that may impact the patient and family’sneeds and initiate intervention by other members of theinterdisciplinary team Initiate communication with the attendingphysician, interdisciplinary team, and other agencies for thepurpose of coordinating optimal care Provide regular visits tore-assess the patient’s needs and update the plan of care Performclinical pronouncement of death and provide support to the familyMaintain accurate patient records, charts, progress notes, reportsand prepare care plans timely Supervise and document delivery ofcare by the Home Health Aide as required by Federal, State, andCompany guidelines
Qualifications:
Graduate from an accreditedregistered nursing program Licensed as a registered nurse in thestate of practice Must possess current CPR, First Aid CertificationMust be knowledgeable of quality assessment and assuranceprocedures Valid driver’s license and proof of insurance isrequired This is your opportunity to do work that truly matters —right now.
