Looking for a Regional Home Care Nurse Job Description The Regional Home Care Nurse is responsible for delivering high-quality, compassionate nursing care to patients in their homes, as referred by partner organizations or internal programs. This role requires strong clinical judgment, the ability to work independently, and a patient-centered approach. The nurse will conduct routine visits to perform wound care, administer medications, and complete general health assessments. Responsibilities Conduct routine home visits to deliver clinical services, including wound care management and dressing changes. Administer and reconcile medications. Perform general health and functional assessments as directed. Provide nursing care in accordance with individualized care plans, physician orders, and best clinical practices. Assess patient conditions and monitor changes in health status, promptly communicating updates to the care team and physician as needed. Collaborate with partner organizations, care coordinators, and internal departments to meet patient needs. Educate patients and caregivers on condition management, medication adherence, wound care, and safety in the home environment. Ensure timely and accurate documentation using the organization’s electronic health record system. Adhere to all clinical protocols, regulatory standards, and infection control guidelines. Participate in interdisciplinary case conferences and clinical meetings as needed. Travel between patient homes throughout the designated service areas: Bronx, Westchester, and Upper Manhattan. Essential Skills Registered Nurse (RN) with a current active and unrestricted license in New York State. Degree from an NLN-accredited nursing school (BSN preferred). At least 3 years of clinical nursing experience, including 1-2 years in home care or a similar healthcare setting preferred. Strong clinical assessment and critical thinking abilities. Excellent communication skills. Capacity to manage multiple participant cases effectively. Proficiency with computers and electronic health record (EHR) systems. Additional Skills & Qualifications Supervisory and management skills. Experience with long-term care or managed care systems is beneficial. Care management and case management experience. Vehicle and current New York State driver’s license preferred. Availability for weekends/nights coverage as needed. Ability to travel to participants’ homes and work in varying environmental conditions. Work Environment The position involves working onsite at a community healthcare clinic located in Westchester and Bronx. The work schedule is from 8:30 AM to 5:30 PM, with a standard 40-hour workweek. The role includes travel within designated areas, and the work environment requires adaptability to varying conditions in patients’ homes. Dress code is professional attire.
