Rate of Pay:
$55.00 – $57.69/hr. depending on degree and experience
Hours:
Monday to Friday 9AM-5PM (35hrs)
Flexible hours, No Nights, Weekends or Holidays!
WORK INDEPENDENTLY!
Travel Requirements:
50-80% Agency Vehicle provided or mileage reimbursement if you use your vehicle.
Position Summary:
Do you want to make a positive difference in the lives of people that are challenged with mental illness and substance use disorders? We have an exciting new opportunity to work for an Assertive Community Treatment (ACT) team that offers treatment, rehabilitation, andmunity integration services to individuals diagnosed with serious mental illness (SMI). ACT uses a person-centered, recovery-based approach to care. Nurses can play a critical part in the recovery and wellness of individuals receiving ACT services. The ACT RN will work on a multi-disciplinary mobile team including psychiatry, nursing, social work, family and peer support and professionals with other types of expertise, such as substance abuse treatment and vocational rehabilitation, and provideprehensive recovery services to adults with serious mental illness throughout Westchester County and the surrounding areas.
Core Responsibilities:
Conduct home andmunity s. Complete health assessments. Provide health education and psychoeducation regarding mental health disorders and treatment modalities
Administer oral and injectable medications
Instruct participants on medication adnce. Support medication management.
Coordinate laboratory services and reports. Collaborate with psychiatrist, clinicians, and other team members. Supervise nursing team members.
Attend clinical and administrative team meetings.
Carry a small caseload of program participants as needed.
Take on-call shifts in rotation with other team members. Supervision and training provided.
Qualifications:
Current NYS Registered Nurse. Valid NYS driver’s license.
Travel Requirements:
50-80% EOE
