The RN Case Manager performs comprehensive assessments, discharge planning, and utilization review for hospitalized patients to ensure safe, timely transitions of care and to reduce length of stay, prevent readmission, and minimize denials. Responsibilities Assess patient needs and develop individualized care plans that align with clinical guidelines. Coordinate with multidisciplinary teams to facilitate effective patient care transitions. Monitor patient progress and adjust care plans as necessary to meet evolving health needs. Provide education and support to patients and their families regarding treatment options and health management. Conduct thorough evaluations of patients with complex medical conditions, including those requiring ICU or critical care. Manage cases involving infectious disease care and ensure adherence to safety protocols. Collaborate with healthcare providers at Level I Trauma Centers to ensure comprehensive patient management. Engage in discharge planning processes to optimize post-hospitalization outcomes. Qualification Registered Nurse (RN) with active New York State license PRI Certification (to be obtained within 3 months of hire) 2 years of clinical experience in ED, critical care, or med/surg. Proven experience using Milliman/InterQual criteria Discharge experience is required.
