You will deliver high-quality nursing services, supporting recovery from surgery, managing chronic conditions, and working to prevent re-hospitalizations. What You’ll Do:

Provide Skilled Post-Acute Home Care Conduct skilled home visits for post-acute care patients across your assigned territory (covering various areas, specific availability discussed upon application). Provide comprehensive nursing care , which includes: Completion of all required documentation. Direct communication with Primary Care Physicians (PCPs). Completion of 485 forms and all interim orders. Updating medication profiles. Care coordination with other disciplines (e.g., physical and occupational therapists, home health aides, social workers). Review and completion of case communication notes. Daily review of follow-up items and incomplete documentation noted in McKesson. Participation in case conferencing with your respective supervisor. Utilize HomeCare HomeBase as the Electronic Medical Record (EMR) system for all documentation.

Schedule & Location Work Week:

Monday-Friday, 9:00 AM

• 5:00 PM.

Setting:

Field Home Visits in

Westchester County Office/Training Location:

5923 Strickland Ave, Brooklyn, NY 11234.

Training:

A mandatory 5-week training program is required for all new hires, regardless of their living location.

This includes:

2 days a week in the Brooklyn office for classroom-based learning. 3 days a week in the field , shadowing in your assigned territory.

Who You Are:

Credentials & Qualifications Registered Nurse (RN) New York State License. Acute Hospital Experience is required. Must possess a Car and a valid Driver’s License. Must be comfortable commuting to Brooklyn for 5-6 weeks for training.

Compensation & Benefits Salary:

$105,000 to $125,000 based on

Experience Bonus:

When applicable, paid out after 15 months of employment. Additional FFS (Fee-For-Service) opportunities are available upon request for full-time candidates.
