We are seeking a compassionate and experienced RN Case Manager to lead and oversee the operations of our Pennsylvania Home Health Division. This is an exciting opportunity for a licensed RN with strong leadership skills and a passion for delivering high-quality, patient-centered care.
Please note:
This is currently a stipend-based position during the initial development phase, until our Pennsylvania office officially opens. This is a unique opportunity to help shape and launch a growing division from the ground up, with the potential for a full-time leadership role upon office opening.

Key Responsibilities:
Lead and manage the daily operations of the home health division, including staffing, budgeting, and patient care oversight Ensure full compliance with all state and federal home health regulations Develop and implement clinical policies and procedures to uphold high standards of care Collaborate with physicians, social workers, and other healthcare professionals to coordinate care plans Oversee patient admissions, discharges, and monitor overall well-being Manage billing, budgeting, and financial reporting functions
What We’re Looking For:
Active Pennsylvania Registered Nurse License or compact licensure Prior experience in home health care or a related field is preferred Proven leadership and staff management experience Strong knowledge of medication administration, long-term care, and regulatory compliance Excellent communication and organizational skills Familiarity with patients’ rights and care coordination
Schedule:
