What You Will Do:
Performs physical and psychosocial assessment on all hospice patients. Identifies hospice diagnosis and documents continuing eligibility. Provides skilled nursing care. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques and symptom control. Provides direct patient care, pain/symptom management to patients as ordered in the patients plan of care to maintain the highest level of comfort and quality of life. Educates patient and patients family regarding death and dying, individual responses to death and patient rights. Develops and coordinates a plan of care for each patient. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Offers ongoing support and palliative care. Initiates needed referrals to other disciplines and/or community resources. Supervise hospice aides as required by the Medicare regulations. Previous hospice experience is highly preferred.
About You:
Must be a Registered Nurses (RN) in the state of Pennsylvania; must be in good standing at the time of hire and renewal. One-year of recent hospice experience preferred. Superior customer service skills. Excellent communication and critical thinking skills are a must. Travel is required throughout the majority of the day. Candidate be willing to operate personal car necessitated by nature of job. Current Driver’s License and car insurance. Able to communicate effectively in English, both verbally and in writing. The position requires standing, sitting, walking, bending, kneeling, stooping, crouching, reaching, and climbing throughout most of the day. The employee must frequently lift or move objects and patients weighing over 50lbs. Routinely transfers patient weight 100lbs or greater. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising patient care.
