Job Description:

We are seeking a dedicated and compassionate Registered Nurse (RN) for Home Health services in the Feasterville-Trevose, PA area. Responsibilities:

Provide skilled nursing care to patients in their homes Conduct patient assessments and develop individualized care plans Collaborate with physicians and interdisciplinary team members Administer medications and treatments as prescribed Educate patients and families on health conditions and care management Maintain accurate clinical documentation

Requirements:

Active RN license in the state of Pennsylvania Minimum of 1 year of recent nursing experience (Home Health preferred) Strong clinical assessment and critical thinking skills Reliable transportation and valid driver’s license Ability to work independently and manage a case load
