PURPOSE AND SCOPE

This position is accountableand responsible for the provision and coordination of clinicallycompetent care including assessment, planning, intervention andevaluation for an assigned group of patients. This may includedelegation of appropriate tasks to direct patient care staffincluding but not limited to RNs, LVN/LPNs and Patient CareTechnicians. As a member of the End Stage Kidney Disease (ESKD)health care team, this position participates in decision-making,teaching, leadership functions, and quality improvement activitiesthat enhance patient care outcomes and facility operations.

PRINCIPAL DUTIES AND RESPONSIBILITIES

All duties andresponsibilities are expected to be performed in accordance withClient policy, procedures, standards of nursing practice, state andfederal regulations. Performs all essential functions under thedirection of the Supervisor and with guidance from the ClinicalEducator, Preceptor or in collaboration with other RegisteredNurses. Performs ongoing, systematic collection and analysis ofpatient data pre

  • post hemodialysis treatment forassigned patients and documents in the patient medical record,makes adjustments or modifications to treatment plan as indicatedand notifies Team Leader, Charge Nurse, Supervisor or Physician asneeded.

Assesses, collaborates, and documents patient/family’sbasic learning needs to provide initial and ongoing education topatients and family. Directs and provides, in collaboration withdirect and ancillary patient care staff, all aspects of the dailyprovision of safe and effective delivery of hemodialysis therapy toassigned patients. Administers medications as prescribed or inaccordance with approved algorithm(s), and documents appropriatemedical justification and effectiveness. Initiates or assists withemergency response measures. Serves as a resource for health careteam, participates in staff training and orientation of new staffas assigned. Ensures correct laboratory collection, processing andshipping procedures are performed and reschedules missed orinsufficient laboratory collections. Identifies expected outcomes,documents and updates the nursing assessment and plan of care forassigned patients through collaboration with the InterdisciplinaryTeam. Ensures patient awareness related to transplant and treatmentmodality options. Performs all other duties as assigned bySupervisor.

PHYSICAL DEMANDS AND WORKING CONDITIONS

The physicaldemands and work environment characteristics described here arerepresentative of those an employee encounters while performing theessential functions of this job. The position provides directpatient care that regularly involves heavy lifting, moving ofpatients and assisting with ambulation. Equipment aids and/orcoworkers may provide assistance. This position requires frequent,prolonged periods of standing and the employee must be able to bendover. The employee may occasionally be required to move, withassistance, machines and equipment of up to 200 lbs., and may liftchemical and water solutions of up to 30 lbs. as high as 5 feet.

The work environment is characteristic of a health care facilitywith air temperature control and moderate noise levels. May beexposed to infectious and contagious diseases/materials. Day to daywork includes desk work, computer work, interaction with patients,facility/hospital staff and physicians. The position may requiretravel to training sites or other facilities. May be asked toprovide essential functions of this position in other locationswith the same physical demands and working conditions as describedabove. Candidates are required to take the Ishihara’s ColorBlindness test as a condition of employment.

SUPERVISION

Assigned oversight of Patient CareTechnicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse inCharge, after meeting all the following: Successful completion ofall client education and training requirements for new employees.

Must have a minimum of 9 months experience as a RN. Must have aminimum of 3 months experience in chronic/acute hemodialysis as aRN. EDUCATION and

LICENSURE

Graduate of an accredited School ofNursing. Current appropriate state licensure. Current or successfulcompletion of CPR BLS Certification. Must meet the practicerequirements in the state in which he or she is employed.

EXPERIENCE AND REQUIRED SKILLS

1 year of Nephrology Nursingexperience in the last two years as a RN.

Chronic hemodialysisexperience required Floating requirement:

Client requiredcandidates to float to area sites within 50 miles of the main siteShift details: Shifts will be discussed in detail during theinterview; the role requires a commitment of 40 hours plus overtimeand floating to area sites as need be. There is a minimum guaranteeof 36 hours per week. Please note you are NOT eligible forGuaranteed hours if any of the following happen: /t
